If you have been to see us recently on the NHS, you should have received letters in November 2020 and January 2021 letting you know that our NHS provision will stop from 1st March 2021. Read more here

We asked patients to let us know by 18th February 2021 if they’d like to stay with us for appointments in future. If you haven’t yet been in touch about this, your appointments from 1st March 2021 will be cancelled.

Transferring your records to a new dentist (Subject Access Request)

A new dentist wouldn’t always need access to your previous records. However, if needed, we can give you copies of your records to take to a new dentist by completing a Subject Access Request form.

If you would like to do this, please get in touch, and we’ll let you know what information we’ll need. You will need to provide us with a copy of photo ID (sending a scanned copy is fine)

It can take up to 30 days for us to process the request. There wouldn’t usually be a charge.

  • Dental Clinical records and notes
  • X-rays
  • Medical details
  • Appointment history
